Costs

The fee for the Reversal is $6,000 which covers pre-operative lab work, surgeon’s fee, anesthesia fee, operating room and post-operative care through the evening of the surgery. However, the cost of the On-Q pain pump (if requested) and the treatment of any unexpected medical complications are not covered by this fee. The cost of any unexpected medical complications may be covered by your medical insurance, even if the Reversal is not.  Semen analysis is available for an additional fee.

A $1000 deposit is required to schedule the surgery. The balance of $5000 is due in full two weeks prior to the procedure. If canceled, $200 of the deposit is non-refundable. $800 will be refunded to you providing two weeks notice is given. If less than two weeks notice is given the entire deposit is forfeited.

Insurance and Financing

We are providers for many managed care plans and health maintenance organizations and, for your convenience, we gladly file your insurance claims for you.

If you do not find Dr. Rosenfeld in your Provider Directory please call as these plans change daily.

We also take Visa, MasterCard, Discover, AMEX and checks. To pay online at any time, fill out our online payment form and a receipt will be emailed to you within one business day.
