The Southwest Center for Microsurgical Tubal Reversals office is open Monday through Friday from 9:00 a.m. to 5:00 p.m.

New patient appointments for tubal ligation reversal surgery can be made by completing the patient registration form online (on rosenfeldmd.com) and a staff member will contact you to schedule an appointment.  The form has four pages and takes fifteen minutes to complete - once complete, press the submit button to securely send your information to our staff.  You will not be able to enter partial information on the form and return later.
