The Southwest Center for Microsurgical Tubal Reversals office is open Monday through Friday from 9:00 a.m. to 5:00 p.m. Wednesday and Friday until 6:00 p.m. Keeping the busy woman in mind, we can also schedule later appointments on Wednesdays and Fridays.

New patient appointments for tubal ligation reversal surgery can be made by completing the patient registration form online (on rosenfeldmd.com) and a staff member will contact you to schedule an appointment.  The form has four pages and takes fifteen minutes to complete - once complete, press the submit button to securely send your information to our staff.  You will not be able to enter partial information on the form and return later.
